How to code Hyperlipidemia

E78.5 (hyperlipidemia, unspecified) is the default when the provider documents hyperlipidemia or dyslipidemia without specifying the lipid fraction. More specific codes are available and should be used when the documentation supports them: E78.00 pure hypercholesterolemia, E78.1 pure hyperglyceridemia (hypertriglyceridemia), E78.2 mixed hyperlipidemia, E78.3 hyperchylomicronemia, E78.41 elevated lipoprotein(a), and E78.49 other hyperlipidemia.

Familial hypercholesterolemia is coded to E78.01-: E78.010 homozygous, E78.011 heterozygous, and E78.019 unspecified. Add Z83.42 for a family history of familial hypercholesterolemia when it is documented. Hypercholesterolemia documented together with hypertriglyceridemia indexes to E78.2 (mixed hyperlipidemia); do not report E78.00 and E78.1 together.

Hyperlipidemia is coded on its own; it is not a combination code with atherosclerosis. When coronary artery disease is also documented, code I25.10 (or the appropriate I25 code) separately. Long-term statin therapy is reported with Z79.899 (other long term current drug therapy). Z13.220 is the screening code for a lipid panel in a patient with no diagnosis; once hyperlipidemia is established, the E78 code is used for monitoring visits instead. Lipoprotein deficiency (E78.6) and the disorders of bile acid and cholesterol metabolism in E78.7- are rare metabolic conditions and not used for routine dyslipidemia.
