Coding Guidance for Sepsis
For sepsis without a documented organism, assign A41.9 (sepsis, unspecified organism). When cultures identify the organism, use the specific code from A40 (streptococcal) or A41 (other organisms) instead.
Severe sepsis requires a minimum of two codes: the underlying systemic infection first, then R65.20 (severe sepsis without septic shock) or R65.21 (with septic shock), plus codes for any documented acute organ dysfunction.
The term urosepsis is not codable in ICD-10-CM; query the provider for clarification. Postprocedural sepsis and sepsis in pregnancy have their own chapter-specific codes and sequencing rules.
