Category J44 has three main branches: J44.0 (COPD with acute lower respiratory infection), J44.1 (COPD with acute exacerbation), and J44.9 (uncomplicated). For J44.0, code the specific infection (such as pneumonia) as well.
An acute exacerbation is a worsening of the chronic condition, not an infection. If both an exacerbation and a lower respiratory infection are documented, both J44.0 and J44.1 may be assigned.
Use an additional code to report tobacco exposure: Z87.891 for personal history of nicotine dependence, F17.- for current use, or Z77.22 for environmental tobacco smoke exposure.
