Coding Guidance for GERD

K21.9 (GERD without esophagitis) is the most frequently assigned code and the default when esophagitis is not documented. K21.0x is used when reflux esophagitis is confirmed, with the sixth character reporting whether bleeding is present (K21.00 without, K21.01 with).

Barrett's esophagus, a possible complication of chronic GERD, is coded separately in category K22.7 and should not be reported with an esophagitis code unless both are documented.
