Coding Guidance for Diabetes Mellitus
Diabetes is organized into five categories by cause: E08 (due to an underlying condition), E09 (drug- or chemical-induced), E10 (Type 1), E11 (Type 2), and E13 (other specified). Type 2 (E11) is the default when the type is not documented.
These are combination codes: the 4th, 5th, and 6th characters identify the complication (for example E11.22 for Type 2 diabetes with diabetic chronic kidney disease). Assign as many codes from one category as needed to describe every documented complication.
Add Z79.4 for long-term insulin use or Z79.84 for oral hypoglycemic drugs when applicable. Do not assign Z79.4 for Type 1 diabetes, where insulin is assumed.
