Coding Guidance for Chronic Kidney Disease
Category N18 is staged: N18.1 through N18.5 for stages 1 to 5, with stage 3 subdivided into N18.30 (unspecified), N18.31 (stage 3a), and N18.32 (stage 3b). N18.6 is end stage renal disease, and N18.9 is unspecified CKD.
Assign N18.6 only when the provider documents ESRD. If a patient has ESRD and is on dialysis, add Z99.2 (dependence on renal dialysis). When both a stage and ESRD are documented, code only N18.6.
CKD frequently appears in combination coding: diabetic CKD uses a diabetes combination code (such as E11.22) followed by the N18 stage, and hypertensive CKD is coded with I12 or I13 first, then the N18 stage.
