Coding Guidance for Hypertension
I10 covers essential (primary) hypertension with no stated organ involvement. When hypertension occurs with heart disease, chronic kidney disease, or both, use the combination categories I11, I12, and I13 instead.
Per the official guidelines, a causal relationship is presumed when a patient has both hypertension and CKD or heart conditions classified to I50 or I51.4 to I51.9. Code the combination category even if the provider does not explicitly link them, unless another cause is documented.
I12 and I13 require an additional code for the CKD stage (N18.-). Hypertensive crisis, urgency, and emergency are reported with category I16, coded alongside the underlying hypertension code.
