Coding Guidance for Stroke (Cerebral Infarction)
Category I63 covers cerebral infarction caused by thrombosis, embolism, unspecified occlusion, or cerebral hemorrhage. I63.9 (cerebral infarction, unspecified) applies when the cause is not documented.
I65 and I66 report occlusion and stenosis of precerebral and cerebral arteries that have not resulted in cerebral infarction, such as asymptomatic carotid stenosis. Use I63 only when an infarction has occurred.
Late effects of stroke, including residual deficits such as aphasia or hemiplegia after healing of the infarct, are coded from category I69 with the original I63 code. A history of resolved cerebral infarction without deficits uses Z86.73 instead.
