Coding Guidance for Pneumonia
J18.9 (pneumonia, unspecified organism) is the default when no causative organism is documented, but always check the record for culture results first: organism-specific codes in J12 to J15 are more specific and affect DRG assignment.
Aspiration pneumonia is coded to J69.0, not to the J12-J18 range. Pneumonia in diseases classified elsewhere (J17) requires the underlying disease to be coded first.
For inpatient reimbursement, simple pneumonia groups to MS-DRG 193, 194, or 195 depending on complications and comorbidities.
