How to code Vitamin D Deficiency

E55.9 (vitamin D deficiency, unspecified) is the code for the great majority of encounters, including documentation of vitamin D insufficiency and low vitamin D. E55.0 is active rickets, the pediatric bone manifestation of the deficiency, and E64.3 is sequelae of rickets in a patient with residual deformity after the deficiency has resolved.

In adults, symptomatic bone softening from vitamin D deficiency is osteomalacia, coded to M83.- in chapter 13 (M83.9 when the type is not specified), with E55.9 added for the deficiency itself when the provider documents both. Osteoporosis (M81.-) is a separate diagnosis and is not assumed from a low vitamin D level. Dietary calcium deficiency documented alongside the vitamin D deficiency is E58.

A vitamin D level on its own is not a diagnosis; the provider must document the deficiency. Z13.21 (encounter for screening for nutritional disorder) is used when the test is ordered for an asymptomatic patient with no known deficiency. Once the deficiency is established, E55.9 supports repeat testing and supplementation follow-up visits. Do not report Z79.899 for over-the-counter vitamin D supplementation unless the provider documents it as long-term drug therapy.
