Thrombocytopenia ICD-10-CM codes

D69.6 (thrombocytopenia, unspecified) is the code for a documented low platelet count with no stated cause. Immune thrombocytopenic purpura (ITP) is D69.3. Primary thrombocytopenia of other types is D69.4-, including D69.41 Evans syndrome and D69.42 congenital and hereditary thrombocytopenic purpura. D69.5- is secondary thrombocytopenia: D69.51 for posttransfusion purpura and D69.59 for thrombocytopenia due to another cause such as chemotherapy, liver disease, sepsis, or a drug. For D69.59, code the underlying condition as well, and add the T36 to T50 adverse effect code with a fifth or sixth character of 5 when a drug is responsible.

Heparin-induced thrombocytopenia is not in D69. It is coded to D75.82-: D75.821 non-immune HIT, D75.822 immune-mediated HIT, and D75.829 HIT unspecified, with T45.515A for the adverse effect of anticoagulants. Thrombocytopenia occurring as part of pancytopenia after chemotherapy is captured by D61.810 (antineoplastic chemotherapy induced pancytopenia) and should not be separately coded to D69.59; D61.818 is other pancytopenia.

A platelet count on a lab report is not a diagnosis; the provider must document thrombocytopenia. When thrombocytopenia is the expected result of the condition being treated, for example in acute leukemia, it is still reported when it is evaluated or affects management. Bleeding manifestations such as epistaxis or gastrointestinal hemorrhage are coded separately. The other D69 codes cover purpura that is not thrombocytopenic: D69.0 allergic purpura (IgA vasculitis), D69.1- qualitative platelet defects (D69.11 Glanzmann thrombasthenia, D69.19 other), and D69.2 other nonthrombocytopenic purpura.
