How to code SIRS (Systemic Inflammatory Response Syndrome)
R65.1- is reserved for SIRS of non-infectious origin, such as SIRS after trauma, burns, or pancreatitis. R65.10 is without acute organ dysfunction and R65.11 is with acute organ dysfunction. The underlying condition is sequenced first, for example K85.90 for acute pancreatitis or an injury code, and R65.1- is never the principal diagnosis.
With R65.11, add codes for each organ dysfunction, such as N17.9 acute kidney injury or J96.00 acute respiratory failure. If an infection is the cause, R65.1- is not used: the case is coded as sepsis, for example A41.9 with R65.20 when there is organ dysfunction.
