How to code Pancytopenia
D61.818 is other pancytopenia and is the default code for acquired pancytopenia with no stated cause. Pancytopenia due to antineoplastic chemotherapy is D61.810, and pancytopenia caused by other drugs is D61.811. With either drug-induced code, add the adverse effect code for the drug, such as T45.1X5A for antineoplastic agents.
Do not report separate codes for the individual cytopenias, such as D69.6 thrombocytopenia or D70.1 neutropenia, when pancytopenia is documented, because the pancytopenia code already covers them. Congenital pancytopenia is D61.09 and Fanconi anemia is D61.03.
Pancytopenia due to myelodysplastic syndrome is coded to the MDS category, for example D46.9, not D61.81-. Aplastic anemia is a different diagnosis; D61.9 is aplastic anemia, unspecified.
