How to code Opioid Use Disorder

F11 follows the same structure as the other substance categories: F11.1- abuse, F11.2- dependence, F11.9- use unspecified. Opioid use disorder documented as moderate or severe is coded to dependence (F11.20 uncomplicated). Patients on methadone or buprenorphine maintenance are coded F11.20 unless the provider documents remission, in which case F11.21 applies; maintenance therapy by itself does not establish remission, because the guidelines allow remission codes only on provider documentation.

Withdrawal is a single code at each level: F11.13 (abuse with withdrawal), F11.23 (dependence with withdrawal), and F11.93 (unspecified use with withdrawal). Intoxication codes carry a sixth character for uncomplicated (F11.220), delirium (F11.221), perceptual disturbance (F11.222), or unspecified (F11.229). Induced mood, psychotic, sleep, and sexual disorders use the 4, 5-, and 8- endings.

Long-term prescribed opioid therapy without an opioid use disorder is Z79.891 (long term current use of opiate analgesic), never an F11 code. Overdose is a poisoning code from T40.- (for example T40.2X1A accidental poisoning by other opioids, initial encounter) sequenced first, with any F11 code as an additional diagnosis. An adverse effect of a properly prescribed opioid is T40.2X5A with the manifestation coded first.
