How to code Lymphoma

Lymphomas occupy C81 to C85 and are coded by histologic type, not by the organ where the mass is found. C81 is Hodgkin lymphoma (C81.1- nodular sclerosis, C81.2- mixed cellularity, C81.0- nodular lymphocyte predominant, C81.9- unspecified). C82 is follicular lymphoma with the grade in the fourth character. C83 is non-follicular lymphoma and includes C83.0- small cell B-cell, C83.1- mantle cell, C83.3- diffuse large B-cell lymphoma (the most common NHL, C83.30 when the site is unspecified), and C83.5- lymphoblastic. C84 covers mature T-cell and NK-cell lymphomas, and C85.9- is non-Hodgkin lymphoma with no type documented.

The fifth character in every lymphoma code is the site: 0 unspecified site, 1 lymph nodes of head, face and neck, 2 intrathoracic, 3 intra-abdominal, 4 axilla and upper limb, 5 inguinal and lower limb, 6 intrapelvic, 7 spleen, 8 lymph nodes of multiple sites, and 9 extranodal and solid organ sites. Lymphoma involving the stomach or skin, for example, uses the ninth character 9, not a C16 or C44 code. Lymph node involvement in lymphoma is part of the disease and is never coded as C77 secondary neoplasm of lymph nodes.

ICD-10-CM has no remission characters for lymphoma (unlike leukemia and myeloma). A patient documented as in remission but still under surveillance or maintenance treatment keeps the lymphoma code. Assign Z85.71 (history of Hodgkin lymphoma) or Z85.72 (history of non-Hodgkin lymphomas) only when the provider documents that the disease has been eradicated and no treatment is directed at it. When the admission is solely for chemotherapy, Z51.11 is principal and the lymphoma code follows.
