How to code Interstitial Lung Disease

J84.9 is interstitial pulmonary disease, unspecified, and is the code for ILD NOS. Idiopathic pulmonary fibrosis is J84.112, nonspecific interstitial pneumonitis is J84.113, and cryptogenic organizing pneumonia is J84.116. Pulmonary fibrosis without a stated type is J84.10; see the pulmonary fibrosis guide.

Drug-induced ILD is coded from J70.2 (acute), J70.3 (chronic), or J70.4 (unspecified), with the drug's adverse effect code added. Hypersensitivity pneumonitis is coded from J67, for example J67.9 when the dust is not specified.

ILD from a connective tissue disease uses the combination code for that disease, such as M34.81 for systemic sclerosis, M05.10 for rheumatoid lung disease, or M35.02 for Sjogren syndrome. Progressive fibrosing ILD in these diseases adds J84.170. Pulmonary sarcoidosis is D86.0.
