How to code Hyperparathyroidism
Primary hyperparathyroidism is E21.0. Secondary hyperparathyroidism is split by cause: N25.81 when it is of renal origin (due to chronic kidney disease), and E21.1 for other causes such as vitamin D deficiency or malabsorption. E21.1 has an Excludes1 note for the renal form, so the two are never reported together.
With N25.81, also code the stage of chronic kidney disease (N18.-), and Z99.2 when the patient is on dialysis. Tertiary hyperparathyroidism is E21.2 and E21.3 is unspecified. Hypercalcemia from primary hyperparathyroidism can be reported with E83.52 when the provider documents it.
