How to code Vertigo and Dizziness

Dizziness, lightheadedness, and vertigo documented without a vestibular diagnosis are all R42 (dizziness and giddiness). Once the provider names a vestibular disorder, use category H81 instead of R42. Benign paroxysmal positional vertigo is H81.1- with laterality: H81.10 unspecified ear, H81.11 right, H81.12 left, H81.13 bilateral. BPPV is frequently coded H81.10 because the affected ear is not documented; the Dix-Hallpike result usually identifies it.

Meniere's disease is H81.0- (H81.01 right, H81.02 left, H81.03 bilateral, H81.09 unspecified). Vestibular neuronitis is H81.2-. Other peripheral vertigo is H81.3- (aural vertigo H81.31-, other peripheral vertigo H81.39-), and vertigo of central origin is H81.4, which has no laterality. H81.9- is an unspecified vestibular disorder and H81.8X- covers other vestibular disorders.

Labyrinthitis is H83.0- (H83.01 right, H83.02 left, H83.03 bilateral, H83.09 unspecified). Vertigo that is a manifestation of another disease is H82.- (vertiginous syndromes in diseases classified elsewhere), with the underlying disease coded first. Syncope (R55) and unsteadiness on feet (R26.81) are distinct symptoms and are not coded as vertigo.
