How to code Vaginitis and Vulvovaginitis

Non-specific vaginitis is category N76: N76.0 acute vaginitis, N76.1 subacute and chronic vaginitis, N76.2 acute vulvitis, N76.3 subacute and chronic vulvitis, N76.4 abscess of vulva, N76.5 and N76.6 ulceration of vagina or vulva, and N76.8- other specified inflammation. Bacterial vaginosis indexes to N76.0, and a B96.89 organism code may be added when the provider documents Gardnerella. Vulvovaginitis without an organism is also N76.0.

When a specific organism is identified, the infection is coded in chapter 1 instead of N76. Vaginal yeast infection is B37.31 (acute candidiasis of vulva and vagina) or B37.32 (chronic candidiasis), a split introduced in FY2023. Trichomonal vulvovaginitis is A59.01. Herpes vulvovaginitis is A60.0- and gonococcal or chlamydial infection of the lower genital tract goes to A54.0- or A56.0-. Do not report N76.0 alongside these organism codes for the same episode.

Atrophic vaginitis in postmenopausal women is N95.2, not N76.-. Vulvar pain without inflammation is vulvodynia, N94.81- (N94.810 vulvar vestibulitis, N94.819 unspecified). Vaginitis in pregnancy is sequenced after the chapter 15 code O23.59-.
