How to code Uterine Fibroids (Leiomyoma)

Uterine fibroids are benign neoplasms and sit in chapter 2 rather than chapter 14. Category D25 has four codes by location: D25.0 submucous, D25.1 intramural, D25.2 subserosal, and D25.9 leiomyoma of uterus unspecified. When imaging or the operative report documents fibroids in more than one layer, assign a code for each documented location. D25.9 is the appropriate code when the note says only fibroids or leiomyomata. Other benign uterine tumors that are not leiomyomas are D26.-.

Fibroids are frequently the reason for a bleeding complaint, and the bleeding pattern is coded in addition when documented: N92.0 heavy regular menses (menorrhagia), N92.1 heavy irregular menses, or N93.9 abnormal uterine bleeding unspecified. Iron deficiency anemia from chronic blood loss is D50.0. Pelvic pressure or pain attributed only to the fibroid is not coded separately.

After hysterectomy, a history encounter uses Z90.710 (acquired absence of both cervix and uterus) and the D25 code is dropped. Fibroids complicating pregnancy are sequenced under the chapter 15 code O34.1- first, followed by D25.-. Leiomyosarcoma is a malignancy (C55 or C54.-) and must never be coded to D25.
