How to code Urinary Incontinence and Overactive Bladder

Incontinence codes are chosen by type. N39.3 is stress incontinence for either sex. N39.4- covers the other specified types: N39.41 urge incontinence, N39.42 incontinence without sensory awareness, N39.43 post-void dribbling, N39.44 nocturnal enuresis, N39.45 continuous leakage, N39.46 mixed (stress and urge) incontinence, N39.490 overflow incontinence, N39.491 coital incontinence, N39.492 postural incontinence, N39.498 other specified incontinence. Incontinence documented without a type is R32 in chapter 18, not an N39 code.

Overactive bladder is N32.81. The N39.4- subcategory carries a code-also note for associated overactive bladder, so a patient with urge incontinence and OAB is N39.41 plus N32.81. When OAB is documented with urgency and frequency but no leakage, report N32.81 alone; the symptoms R39.15 (urgency) and R35.0 (frequency) are integral to OAB and are not added unless the provider documents them as separate problems. Nocturia (R35.1) is commonly reported alongside these codes.

Mixed incontinence is one code, N39.46, not N39.3 plus N39.41. Functional incontinence due to mobility or cognitive limitation is R39.81. In women with pelvic organ prolapse, add the N81 code (N81.10 cystocele, N81.4 uterovaginal prolapse) as the underlying cause. Bedwetting in a child without a physical cause is F98.0, a chapter 5 code, rather than N39.44.
