How to code Subdural Hematoma

The first question is whether the subdural hematoma is traumatic. A traumatic subdural hematoma is S06.5X-, with the 6th character for loss of consciousness and the 7th character for the encounter. S06.5X0A is without loss of consciousness, initial encounter, and S06.5X9A is loss of consciousness of unspecified duration. Use D for subsequent encounters and S for sequelae. Add the external cause, such as W19.XXXA for an unspecified fall.

A subdural hematoma with no injury documented is nontraumatic and coded to I62.0-: I62.00 unspecified, I62.01 acute, I62.02 subacute, and I62.03 chronic. A chronic subdural hematoma in an older patient is coded as traumatic only when the provider links it to an injury. Add Z79.01 when the patient is on long-term anticoagulation.

Other intracranial bleeds have their own codes: traumatic epidural hemorrhage is S06.4X-, such as S06.4X0A, traumatic subarachnoid hemorrhage is S06.6X-, such as S06.6X0A, and nontraumatic extradural hemorrhage is I62.1. After treatment is complete, a past traumatic brain injury is Z87.820.
