How to code Schizophrenia

F20.9 (schizophrenia, unspecified) is the code for schizophrenia without a documented subtype and is the correct DSM-5 crosswalk, since DSM-5 no longer uses subtypes. The legacy subtype codes remain valid when documented: F20.0 paranoid, F20.1 disorganized, F20.2 catatonic, F20.3 undifferentiated, and F20.5 residual. Schizophreniform disorder (symptoms under six months) is F20.81.

Schizoaffective disorder is a separate category: F25.0 (bipolar type), F25.1 (depressive type), and F25.9 (unspecified). Delusional disorder is F22, brief psychotic disorder is F23, and F29 is unspecified psychosis not due to a substance or known physiological condition. F29 is the correct code for psychosis NOS on an initial evaluation before a diagnosis is established.

Psychosis caused by a medical condition is coded to F06.0 (with hallucinations) or F06.2 (with delusions), with the underlying condition sequenced first. Substance-induced psychosis uses the substance category (for example F10.259 or F12.959), not F20 or F29. Hallucinations as a symptom without a psychotic diagnosis are R44.3.
