How to code Preterm Labor

Category O60 is split three ways. O60.0- is preterm labor without delivery (O60.02 second trimester, O60.03 third trimester, O60.00 unspecified). O60.1- is preterm labor with preterm delivery, where the fifth character gives the trimester of labor onset and the sixth the trimester of delivery: O60.12 labor and delivery both in the second trimester, O60.13 labor in the second with delivery in the third, O60.14 both in the third. O60.2- is term delivery after an episode of preterm labor.

O60.1- and O60.2- take a seventh character identifying the fetus, with X as a placeholder: 0 for a single gestation or when the fetus is not specified, 1 through 5 for a numbered fetus in a multiple gestation, 9 for other fetus. A singleton delivered in the third trimester after preterm labor is O60.14X0. Preterm means before 37 completed weeks. False labor before 37 weeks is O47.0- (O47.00, O47.02, O47.03), not O60. Premature rupture of membranes is O42.-, coded in addition when present.

Add the Z3A code for weeks of gestation (for example Z3A.34) and, on the delivery record, the Z37 outcome code (Z37.0 single live birth). The infant's prematurity is coded on the newborn record with P07, never on the mother's record.
