How to code Postpartum Hemorrhage

Category O72 is defined by timing. O72.0 is third-stage hemorrhage, bleeding associated with a retained or adherent placenta before it is delivered. O72.1 is other immediate postpartum hemorrhage, within 24 hours after delivery of the placenta, and includes atonic hemorrhage. O72.2 is delayed or secondary hemorrhage after the first 24 hours, often from retained products of conception. O72.3 is postpartum coagulation defects such as afibrinogenemia or fibrinolysis.

Uterine atony without hemorrhage is O62.2, not O72.1. Bleeding during labor before delivery is intrapartum hemorrhage in O67.-, and bleeding from placenta previa or placental abruption is coded to O44 or O45 (O45.90 for unspecified abruption). Placenta accreta, increta, and percreta are O43.21-, O43.22-, and O43.23- by trimester and are reported in addition when they cause the hemorrhage.

Acute blood loss anemia after delivery is reported as O90.81 (anemia of the puerperium) with D62 as the additional code that specifies the anemia. A delivery with postpartum hemorrhage cannot be coded O80; report the O72 code with the Z37 outcome of delivery code and the Z3A weeks of gestation.
