How to code Postpartum Depression

Postpartum depression documented by the provider is F53.0. Puerperal psychosis is F53.1. Both are chapter 5 codes even though the condition arises after delivery, and they stand alone; no O code is required with F53.0 for a depression that began in the puerperium.

O90.6 (postpartum mood disturbance) is for the transient postpartum blues, dysphoria, or sadness that does not meet criteria for a depressive disorder. Do not report O90.6 and F53.0 together for the same episode; the F53.0 code is the more specific diagnosis. Anxiety after delivery without a depressive diagnosis is F41.9 or the specific anxiety disorder.

When a patient with a pre-existing depressive or other mental disorder is treated during pregnancy or the puerperium, the chapter 15 code comes first: O99.34- (O99.343 third trimester, O99.345 puerperium) followed by the F32 or F33 code that describes the disorder, for example F32.A (depression, unspecified) or F32.9. A routine postpartum visit at which depression screening is negative is Z39.2.
