How to code Plantar Fasciitis

Starting October 1, 2026 (FY2027), plantar fasciitis has its own codes with laterality: M67.A01 right foot, M67.A02 left foot, and M67.A09 unspecified foot. Before FY2027 it was indexed to M72.2. There is no bilateral code, so report M67.A01 and M67.A02 together for both feet.

M72.2 is now limited to plantar fascial fibromatosis (Ledderhose disease), a nodular thickening of the fascia rather than an inflammatory heel condition, and it also carries laterality: M72.21 right, M72.22 left, M72.20 unspecified. A calcaneal spur documented on imaging is M77.3-: M77.31 right foot, M77.32 left foot, M77.30 unspecified. Report both M67.A0- and M77.3- when the provider documents both fasciitis and a spur. Other enthesopathies of the foot and ankle, such as insertional problems not otherwise specified, are M77.5-.

Heel pain with no diagnosis established is a symptom code, M79.671 (right foot), M79.672 (left foot) or M79.673 (unspecified), not M67.A0-. Achilles tendinitis is M76.6- and metatarsalgia is M77.4-, both with laterality. Tarsal tunnel syndrome is a nerve entrapment coded to G57.5-.
