How to code Peripheral Neuropathy
G62.9 (polyneuropathy, unspecified) is the default for peripheral neuropathy or polyneuropathy with no documented cause. Specific causes have their own codes: G62.0 drug-induced (add the adverse effect T code, such as T45.1X5A for chemotherapy), G62.1 alcoholic (add F10.-), G62.2 due to other toxic agents, G62.81 critical illness polyneuropathy, and G62.82 radiation-induced. Hereditary neuropathy is G60.- and Guillain-Barre syndrome is G61.0.
Diabetic neuropathy is not coded to G62. The diabetes categories carry combination codes: E11.40 (Type 2 with diabetic neuropathy, unspecified), E11.41 mononeuropathy, E11.42 polyneuropathy, E11.43 autonomic neuropathy, and E11.44 amyotrophy, with parallel codes in E08, E09, E10, and E13. The word with in the index links diabetes and neuropathy, so a patient with diabetes and polyneuropathy is coded E11.42 even if the provider does not write diabetic, unless another cause is documented.
G63 (polyneuropathy in diseases classified elsewhere) requires the underlying disease to be sequenced first and is used for causes such as amyloidosis, vitamin deficiency, or connective tissue disease. Do not pair G63 with a diabetes code, because E11.42 already reports the neuropathy.
Mononeuropathies of a single nerve are coded to G56 (upper limb) and G57 (lower limb) with laterality, and mononeuritis multiplex is G58.7. Numbness or tingling without a neuropathy diagnosis is R20.2 (paresthesia of skin). M79.2 (neuralgia and neuritis, unspecified) is a symptom-level code and should not replace a documented polyneuropathy.
