How to code Pancreatitis

Acute pancreatitis is K85 and the fourth character gives the cause: K85.0- idiopathic, K85.1- biliary (gallstone pancreatitis), K85.2- alcohol induced, K85.3- drug induced, K85.8- other, and K85.9- unspecified. The fifth character reports complications: 0 without necrosis or infection, 1 with uninfected necrosis, and 2 with infected necrosis. Acute pancreatitis with no cause documented is K85.90; with infected necrosis it is K85.92.

Biliary pancreatitis takes an additional K80.5- code for the bile duct stone. Alcohol induced pancreatitis (K85.2- or chronic K86.0) is reported with an F10.- code for alcohol abuse or dependence. Drug induced pancreatitis (K85.3-) requires an adverse effect code from T36 to T50 with a fifth or sixth character of 5. Hypertriglyceridemia (E78.1) is coded in addition when identified as the cause.

Chronic pancreatitis is K86.0 (alcohol induced) or K86.1 (other chronic pancreatitis). An acute flare in a patient with chronic pancreatitis is coded with both the K85 and K86 codes. K86.0 has a code-also note for exocrine pancreatic insufficiency (K86.81), and pancreatic pseudocyst is K86.3.
