How to code Osteopenia (Low Bone Mass)

Osteopenia has no code with its own name. The index sends it to M85.8- (other specified disorders of bone density and structure). M85.80 is the unspecified site, M85.89 is multiple sites, and M85.88 is another site such as the spine. Site codes with laterality are also available, such as M85.851 for the right thigh (femur). Osteopenia found on a DXA scan of the hip and spine is usually reported with M85.89.

Osteopenia is not osteoporosis. When the provider documents osteoporosis, code M81.0 (age-related osteoporosis without current pathological fracture) or the M80 code if there is a fragility fracture, and do not add M85.8-. See the osteoporosis guide for those codes.

Supporting codes are often reported with osteopenia: Z13.820 for a screening DXA, Z78.0 for asymptomatic menopause, Z79.83 for long-term bisphosphonate use, and Z87.310 for a personal history of a healed osteoporosis fracture.
