How to code Osteomyelitis

Osteomyelitis is coded to M86 by type and site. M86.9 is osteomyelitis unspecified. Acute osteomyelitis is M86.0- (hematogenous) or M86.1- (other acute), subacute is M86.2-, and chronic is M86.3- to M86.6-, with M86.4- for chronic osteomyelitis with a draining sinus. Most site codes carry laterality, such as M86.171 for other acute osteomyelitis of the right ankle and foot and M86.671 for other chronic osteomyelitis of the right ankle and foot.

M86.8X- (other osteomyelitis) is used when the type is not acute, subacute, or chronic as described above, for example M86.8X71 or M86.8X72 for the ankle and foot. Osteomyelitis of the spine is not in M86; it is M46.2-, such as M46.26 for the lumbar region.

Diabetic foot osteomyelitis takes the diabetes code first. For type 2 diabetes, report E11.69 (diabetes with other specified complication) with the M86 code for the site. When a foot ulcer is also present, add E11.621 and the L97 ulcer code, such as L97.519. Code the organism with a B95 to B96 code when it is documented.
