How to code Neonatal Jaundice

P59.9 (neonatal jaundice, unspecified) is the code for physiologic jaundice and for neonatal hyperbilirubinemia without a documented cause. P59.0 is jaundice associated with preterm delivery, P59.3 is breast milk jaundice, P59.1 inspissated bile syndrome, P59.2- jaundice from hepatocellular damage, and P59.8 other specified causes. Do not use R17 (unspecified jaundice) for a newborn; the perinatal code applies.

When the jaundice results from hemolysis, use the cause-specific code instead of P59.9: P58.0 due to bruising, P58.1 due to bleeding, P58.2 due to infection, P58.41 or P58.42 due to drugs or toxins, P58.9 unspecified excessive hemolysis. Isoimmunization is category P55: P55.0 Rh isoimmunization, P55.1 ABO isoimmunization, P55.9 unspecified hemolytic disease. Kernicterus is P57.0 (due to isoimmunization) or P57.9.

On the birth admission the Z38 code (Z38.00 vaginal, Z38.01 cesarean, single liveborn in hospital) is the principal diagnosis and the jaundice code is secondary. On a readmission for phototherapy the P59 or P55 code is the principal diagnosis. Chapter 16 codes are reported on the newborn record only, and may be used beyond the neonatal period if the condition is still being treated.
