Coding Guidance for Migraine
Category G43 distinguishes migraine without aura (G43.0), migraine with aura (G43.1), chronic migraine (G43.7), and other specified forms such as hemiplegic migraine. G43.909 (migraine, unspecified, not intractable, without status migrainosus) is the default when details are absent.
The sixth character captures complications: codes ending in 9 are not intractable and without status migrainosus, while intractable migraine and status migrainosus (a debilitating attack lasting more than 72 hours) require their own characters. Both may be documented for the same encounter.
Headache attributed to another condition, such as post-craniotomy headache, is coded elsewhere; use G43 only when the provider documents migraine as the diagnosis.
