How to code Macular Degeneration

Age-related macular degeneration is split into nonexudative (dry) H35.31- and exudative (wet) H35.32-. Both take a sixth character for laterality (1 right, 2 left, 3 bilateral, 9 unspecified) and a seventh character for stage. H35.30 (unspecified macular degeneration) is used when the provider does not state dry or wet.

Dry AMD stages are 0 stage unspecified, 1 early dry, 2 intermediate dry, 3 advanced atrophic (geographic atrophy) without subfoveal involvement, and 4 advanced atrophic with subfoveal involvement. H35.3130 is bilateral dry AMD, stage unspecified, and H35.3132 is bilateral intermediate dry AMD. Wet AMD stages are 0 unspecified, 1 with active choroidal neovascularization, 2 with inactive CNV, and 3 with inactive scar.

Each eye can be at a different stage or type. A patient with wet AMD in the right eye and dry AMD in the left is coded H35.321- and H35.312- with their own stages, not with a bilateral code. Anti-VEGF injection encounters are coded to the wet AMD code with active CNV (for example H35.3211 for the right eye).

Drusen without a diagnosis of AMD is H35.36- (drusen of macula). Visual impairment can be reported in addition with H54.- when the provider documents the level (H54.7 unspecified visual loss, H54.3 unqualified visual loss both eyes).
