How to code Lymphedema

I89.0 (lymphedema, not elsewhere classified) covers acquired lymphedema of any limb, including lymphedema after cancer treatment other than mastectomy. It has no laterality or site codes.

Lymphedema of the arm after breast cancer surgery is I97.2 (postmastectomy lymphedema syndrome), not I89.0. It is often reported with Z90.11 or Z90.12 for the absent breast and Z85.3 for a history of breast cancer, or the C50 code if the cancer is still being treated. Add Z92.3 when radiation is documented. Hereditary or primary lymphedema, such as Milroy disease, is Q82.0.

Swelling not diagnosed as lymphedema is localized edema, R60.0, and leg swelling from venous disease is chronic venous insufficiency, I87.2. Cellulitis in a lymphedematous leg is coded with L03.115 or L03.116 in addition to the lymphedema code.
