How to code Irritable Bowel Syndrome
Category K58 is subdivided by predominant bowel pattern: K58.0 IBS with diarrhea (IBS-D), K58.1 IBS with constipation (IBS-C), K58.2 mixed IBS (IBS-M), K58.8 other IBS, and K58.9 IBS unspecified. The category includes irritable colon and spastic colon. K58.9 is the default when the provider documents IBS with no subtype.
Diarrhea, constipation, bloating, and abdominal pain are integral to IBS and are not coded separately with K58. Do not add R19.7 to K58.0 or K59.00 to K58.1. Functional diarrhea (K59.1) and functional dyspepsia (K30) are distinct functional disorders and are only coded when the provider names them rather than IBS.
IBS is a clinical diagnosis based on symptom criteria. It should not be coded from a plan to rule it out or from a differential; if the provider has not committed to the diagnosis, report the presenting symptoms instead.
