How to code Hypotension
I95.9 is hypotension unspecified and is the default when the provider documents hypotension without a type. I95.1 is orthostatic (postural) hypotension, I95.0 is idiopathic hypotension, I95.3 is hypotension of hemodialysis, I95.81 is postprocedural hypotension, and I95.89 covers other specified forms.
I95.2 (hypotension due to drugs) includes orthostatic hypotension due to drugs. Add an adverse effect code from T36 to T50 with a fifth or sixth character of 5 to identify the medication, for example T46.5X5A for an antihypertensive taken as prescribed. Neurogenic orthostatic hypotension due to multiple system atrophy is classified to G90.3, not I95.1.
A low blood pressure reading without a diagnosis of hypotension is R03.1. Do not use an I95 code for shock: cardiogenic, hypovolemic, and other shock are R57.-, and septic shock is coded with the sepsis rules. Syncope (R55) and dehydration (E86.0) are coded separately when documented alongside hypotension.
