How to code Hip Fracture

Hip fractures are in category S72 and are grouped by anatomic region. S72.0- is the head and neck of the femur (intracapsular): S72.00- unspecified part of neck, S72.01- unspecified intracapsular, S72.03- midcervical, S72.04- base of neck, S72.05- and S72.06- head of femur. S72.1- is the trochanteric region: S72.10- unspecified trochanteric, S72.11- greater trochanter, S72.12- lesser trochanter, S72.14- intertrochanteric. S72.2- is subtrochanteric. The sixth character reports displacement and laterality (S72.141A displaced intertrochanteric right, S72.144A nondisplaced right). S72.001A (unspecified part of neck of right femur) is the default for a femoral neck fracture that the record does not further describe.

S72 uses the full set of seventh characters with the Gustilo classification: A closed initial encounter, B open type I or II, C open type IIIA to IIIC, D through R for subsequent encounters by healing status and open or closed, and S for sequela. Almost every hip fracture in an older adult is closed, so A is the initial encounter character and D the routine follow-up character. Surgical repair during the initial admission is still an A encounter; the D characters begin with post-repair follow-up.

When the fracture results from osteoporosis and the trauma would not break a healthy bone, the guidelines require an M80 code rather than S72: M80.051A for age-related osteoporosis with a current pathological fracture of the right femur. A pathological fracture from another disease is M84.4- (M84.459A hip, unspecified), and a fracture around a hip prosthesis is M97.0- (M97.01XA right). Add the external cause (W01.0XXA slip or trip, W19.XXXA unspecified fall) and Z91.81 (history of falling) when documented.
