How to code Herpes Zoster (Shingles)

B02.9 (zoster without complications) is the code for uncomplicated shingles, which is most outpatient presentations. The fourth and fifth characters add complications: B02.0 encephalitis, B02.1 meningitis, B02.7 disseminated zoster, and B02.8 for other complications such as secondary bacterial infection. Zoster ophthalmicus is coded to B02.3-: B02.30 when the eye involvement is not further specified, B02.31 conjunctivitis, B02.33 keratitis.

Postherpetic neuralgia is coded to B02.2-, not to the neuralgia codes in chapter 6. B02.22 is postherpetic trigeminal neuralgia, B02.23 postherpetic polyneuropathy, B02.21 geniculate ganglionitis (Ramsay Hunt syndrome), and B02.29 for other postherpetic nervous system involvement, which is where generic postherpetic neuralgia lands. Once the rash has cleared and only the pain persists, the B02.2- code alone is reported; do not add B02.9.

Zoster is a reactivation of varicella, so a diagnosis of shingles is never coded to B01 (varicella). An encounter solely for the shingles vaccine is Z23. Pain integral to the acute rash is not coded separately.
