How to code Herpes Simplex

B00.9 is herpesviral infection, unspecified, for an HSV infection with no stated site. Cold sores and herpes labialis are B00.1 (herpesviral vesicular dermatitis), herpetic gingivostomatitis is B00.2, eczema herpeticum is B00.0, and herpetic whitlow is B00.89. Eye involvement is coded from B00.5-, such as B00.52 for herpes keratitis.

Genital herpes is classified to A60 (anogenital herpesviral infection), not B00. A60.04 is herpesviral vulvovaginitis, A60.01 is herpes of the penis, A60.1 covers perianal skin and rectum, and A60.9 is anogenital herpes unspecified. Congenital or neonatal herpes is P35.2.

Serious HSV infections have their own codes: B00.3 herpes meningitis, B00.4 herpes encephalitis, B00.81 herpes hepatitis, and B00.7 disseminated disease. Herpes zoster (shingles) is a different virus and is coded to B02.
