How to code Hepatitis B

B16 covers acute hepatitis B: B16.9 (without delta agent, without hepatic coma) is the acute default, B16.2 adds hepatic coma, and B16.0 and B16.1 apply when the delta agent (hepatitis D) is present. B17.0 is an acute delta superinfection in an existing hepatitis B carrier. Chronic hepatitis B is B18.1 without delta agent and B18.0 with delta agent. B19.10 (unspecified viral hepatitis B without hepatic coma) is used only when the record states neither acute nor chronic.

Chronic hepatitis B carrier, hepatitis B surface antigen positive, and inactive carrier all index to B18.1; ICD-10-CM has no separate carrier status code for viral hepatitis. Associated cirrhosis (K74.6-) and hepatocellular carcinoma (C22.0) are coded in addition. When hepatitis B complicates pregnancy, sequence O98.411 to O98.419 by trimester first, then the specific B code.

Use Z11.59 for screening of an asymptomatic patient with no exposure, and Z23 for a vaccination-only encounter. A patient documented as immune after vaccination has no active or history code; do not assign B18.1 or Z86.19 for immunity. Documentation of hepatitis B with liver failure requires the with-coma code only when hepatic coma or hepatic encephalopathy is explicitly stated.
