How to code Hemorrhoids

K64.9 is hemorrhoids unspecified and includes bleeding hemorrhoids and hemorrhoids without mention of degree. K64.8 (other hemorrhoids) is for internal hemorrhoids documented without a degree and for prolapsed hemorrhoids of unstated degree. External hemorrhoids NOS and anal skin tags index to K64.4, and a thrombosed external hemorrhoid is K64.5 (perianal venous thrombosis).

Internal hemorrhoids graded by the provider use the degree codes: K64.0 first degree (no prolapse), K64.1 second degree (prolapse that reduces spontaneously), K64.2 third degree (prolapse requiring manual reduction), and K64.3 fourth degree (irreducible prolapse). Code the degree exactly as documented and do not infer it from the treatment performed.

Bleeding is included in the hemorrhoid codes, so K62.5 (hemorrhage of anus and rectum) is not added for hemorrhoidal bleeding. Hemorrhoids in pregnancy are excluded from K64 and reported with O22.4- (O22.40 for an unspecified trimester), and hemorrhoids complicating the puerperium are O87.2.
