How to code Heart Valve Disorders

Nonrheumatic valve disorders are coded by valve: I34 mitral, I35 aortic, I36 tricuspid, and I37 pulmonary. Within each category the fourth character identifies stenosis (.0), insufficiency or regurgitation (.1), stenosis with insufficiency (.2), other disorders (.8), and unspecified (.9). Aortic stenosis is I35.0, mitral regurgitation is I34.0, mitral valve prolapse is I34.1, and mitral annular calcification is I34.81.

The rheumatic default differs by valve. Mitral stenosis and mitral valve disease NOS are classified as rheumatic (I05.0 and I05.9) unless the provider documents them as nonrheumatic, while aortic valve disorders default to nonrheumatic I35 unless stated to be rheumatic (I06.-). When both mitral and aortic valves are diseased and the cause is rheumatic or unspecified, use I08.- (I08.0 for mitral and aortic), because I08 includes multiple valve disease specified as rheumatic or unspecified.

I35 has a code-also note for bicuspid aortic valve (Q23.81), so report the congenital anomaly with the stenosis when documented. Endocarditis of an unspecified valve is I38, and acute infective endocarditis is I33.0. After valve replacement, report the status with Z95.2 (prosthetic), Z95.3 (xenogenic or bioprosthetic), or Z95.4 (other replacement), and use T82.0- for complications of the prosthesis rather than a valve disease code.
