How to code Hearing Loss

Category H90 is organized by type and then laterality. Sensorineural hearing loss is H90.3 (bilateral), H90.41 (right ear, unrestricted hearing on the other side), H90.42 (left ear), and H90.5 (unspecified). Conductive is H90.0, H90.11, H90.12, and H90.2. Mixed is H90.6, H90.71, H90.72, and H90.8. H90.3 is the most common code because age-related sensorineural loss is usually bilateral.

The H90.A- codes report unilateral hearing loss when the opposite ear also has restricted hearing (for example H90.A21 sensorineural loss right ear with restricted hearing on the contralateral side). Use them when the provider documents asymmetric loss in both ears rather than assigning two unilateral codes.

Hearing loss with a documented cause or pattern goes to H91: presbycusis H91.1- (H91.13 bilateral), sudden idiopathic hearing loss H91.2- (H91.23 bilateral), ototoxic hearing loss H91.0- with the drug's T code, and H91.9- for unspecified hearing loss (H91.90 unspecified ear, H91.93 bilateral). H91.3 is deaf nonspeaking. Noise-induced hearing loss is coded to the inner ear noise effect, H83.3X-.

Hearing aid use is Z97.4 and cochlear implant status is Z96.21. A hearing examination with no abnormal findings is Z01.10, and follow-up after a failed newborn screen is Z01.110. Speech delay caused by hearing loss is F80.4 with the hearing loss coded as well. Tinnitus (H93.1-) is a separate diagnosis and is coded in addition when documented.
