How to code Gastritis

Category K29 is subdivided by type: K29.0- acute gastritis, K29.2- alcoholic gastritis, K29.3- chronic superficial gastritis, K29.4- chronic atrophic gastritis, K29.5- chronic gastritis unspecified, K29.6- other gastritis, K29.7- gastritis unspecified, K29.8- duodenitis, and K29.9- gastroduodenitis unspecified. Gastritis with no other detail is K29.70.

Every subcategory ends in 0 for without bleeding and 1 for with bleeding, so acute hemorrhagic gastritis is K29.01 and unspecified gastritis with bleeding is K29.71. These are combination codes; do not report K92.2 with them. Gastritis is not the same as gastroenteritis, which is K52.9.

H. pylori gastritis is coded with the K29 code followed by B96.81 for the organism. Alcoholic gastritis (K29.20 or K29.21) is reported with an F10.- code for the alcohol use. Eosinophilic gastritis is excluded from K29 and coded as K52.81. GERD (K21.9) is coded separately when documented alongside gastritis.
