How to code Gallstones and Cholecystitis

K80 codes combine three elements: where the stones are, whether cholecystitis or cholangitis is present, and whether there is obstruction. Gallbladder stones use K80.0- with acute cholecystitis, K80.1- with chronic, acute and chronic, or other cholecystitis, and K80.2- without cholecystitis. Bile duct stones use K80.3- with cholangitis, K80.4- with cholecystitis, and K80.5- alone, and K80.6- and K80.7- cover stones in both the gallbladder and bile duct. The final character reports obstruction, for example K80.20 without and K80.21 with obstruction.

Biliary colic without documented cholecystitis is K80.20 because the K80.2 subcategory includes colic of the gallbladder. Gallstone pancreatitis is K85.1- with the K80.5- code for the duct stone. When the record shows cholecystitis with stones, assign only the K80 combination code; K81 has an Excludes1 note for cholecystitis with cholelithiasis.

Acalculous cholecystitis is K81: K81.0 acute, K81.1 chronic, K81.2 acute on chronic, and K81.9 unspecified. K81 instructs coders to add K82.A1 for gangrene or K82.A2 for perforation of the gallbladder when documented. Cholangitis without stones is K83.09 and gallbladder obstruction without stones is K82.0.

After cholecystectomy, report Z90.49 for the acquired absence of the gallbladder, and a stone found in the duct after surgery is K91.86 (retained cholelithiasis following cholecystectomy), which K80 excludes.
