K76.0 (fatty change of liver, not elsewhere classified) is the code for hepatic steatosis, NAFLD, and the newer term MASLD (metabolic dysfunction-associated steatotic liver disease), which is listed as an inclusion term. Nonalcoholic steatohepatitis (NASH) and its newer name MASH are K75.81, and K76.0 has an Excludes1 note for K75.81, so report one or the other, not both.

Fatty liver due to alcohol is K70.0 with an additional F10.- code for the alcohol use, and it is not coded with K76.0. When the provider does not state whether the steatosis is alcoholic, K76.0 applies.

Fibrosis staging uses K74.0-: K74.00 unspecified, K74.01 early fibrosis, and K74.02 advanced fibrosis. K75.81 instructs coders to add the K74.0- code when fibrosis is documented, so NASH with advanced fibrosis is K75.81 plus K74.02. Once cirrhosis develops, K74.60 (or K74.69) is reported with K75.81 to preserve the etiology.

Code the metabolic drivers when documented: obesity (E66.-), type 2 diabetes (E11.-), and metabolic syndrome (E88.810). These are not assumed from the MASLD diagnosis and must appear in the record.
