How to code Encephalopathy

G93.40 is encephalopathy, unspecified. Metabolic encephalopathy is G93.41, which also covers septic encephalopathy, and G93.49 is other encephalopathy. Septic encephalopathy is reported with the sepsis code first, for example A41.9 followed by G93.41.

Toxic encephalopathy is category G92. G92.9 is unspecified toxic encephalopathy and G92.8 is other toxic encephalopathy, which includes toxic metabolic encephalopathy. For G92.8 and G92.9, a poisoning (T36-T65 with fifth or sixth character 1-4) is coded first, while an adverse effect of a correctly taken drug is added after as T36-T50 with fifth or sixth character 5. Since FY2022, G92.0- is used only for immune effector cell-associated neurotoxicity syndrome (ICANS), graded G92.00-G92.05.

Several encephalopathies have their own codes outside G93.4: hepatic encephalopathy K76.82, hypertensive encephalopathy I67.4, Wernicke encephalopathy E51.2, anoxic brain damage G93.1, and neonatal hypoxic ischemic encephalopathy P91.60. See the hepatic encephalopathy guide for liver-related coding.
