How to code Eating Disorders

Category F50 gained severity and remission characters in FY2024. Anorexia nervosa is F50.01- (restricting type) or F50.02- (binge eating/purging type) with a sixth character for mild (0), moderate (1), severe (2), extreme (3), in remission (4), or unspecified (9). F50.00 is anorexia nervosa, unspecified type. Bulimia nervosa is F50.2- with the same severity scale at the fifth character (F50.20 unspecified through F50.25 in remission).

Binge eating disorder is F50.81- with the same severity structure (F50.810 mild through F50.819 unspecified). Avoidant/restrictive food intake disorder (ARFID) is F50.82, pica in adults is F50.83, rumination disorder in adults is F50.84, and F50.89 is other specified eating disorder. F50.9 is eating disorder, unspecified. Pica and rumination in infancy and childhood use F98.3 and F98.21.

Severity should follow the provider's documented DSM-5 severity, which for anorexia nervosa is based on BMI and for bulimia and binge eating on episode frequency. Do not derive severity from the BMI yourself. Report the BMI Z-code (for example Z68.18 for an adult BMI of 18.4 or less) and any malnutrition diagnosis (E43, E46) as additional codes when documented. R63.0 (anorexia as a symptom) and R63.4 (abnormal weight loss) are symptom codes and are not used once an eating disorder is diagnosed.
