How to code Diverticulitis and Diverticulosis

Category K57 is organized by site and by whether perforation and abscess are present. The fourth character pairs are K57.0 and K57.1 for the small intestine, K57.2 and K57.3 for the large intestine, K57.4 and K57.5 for both, and K57.8 and K57.9 for an unspecified part. The first code of each pair (K57.0, K57.2, K57.4, K57.8) means with perforation and abscess; the second means without.

The fifth character distinguishes diverticulosis from diverticulitis and reports bleeding: 0 diverticulosis without bleeding, 1 diverticulosis with bleeding, 2 diverticulitis without bleeding, and 3 diverticulitis with bleeding. Uncomplicated diverticulitis of the colon is K57.32, colonic diverticulosis found on colonoscopy is K57.30, and a diverticular bleed is K57.31 or K57.33. Diverticulitis with no site documented indexes to K57.92, so confirm whether the provider specified the colon.

Bleeding is built into the code, so K92.2 is not added for diverticular hemorrhage. For perforation with abscess or peritonitis, use the with-perforation-and-abscess code (K57.20 for the colon) and follow the code-also note for peritonitis (K65.-), reporting K65.0 when generalized peritonitis is documented separately.
